2008 Football and Cheer Sign-Ups

Registration for the 2008 Alameda Pirates is now open for "sign-up" to ensure your spot for the upcoming football and cheer season.  THE LAST DAY TO SUBMIT FORMS IS FRIDAY, AUGUST 8TH

Form Deadlines:

Code of Conduct	July 21, 2008
Equipment Policy / Deposit	July 25, 2008
Physical	July 28, 2008
Birth Certificate	August 8, 2008

Required forms can be download below.   Bring the completed registration package to any football practices (registration package are also available at the practice) going on Monday thru Friday from 6p - 8p (until September) at College of Alameda.  Or, you can mail the completed package to Alameda Pirates, PO Box 642, Alameda, Ca 94501. Players and Cheers will need the following documents to complete their registration:

• Certified County Birth Certificate

• Current Physical (Dated and Stamped in 2008)

• Equipment Deposit of $350.00 (Football Only-Check will NOT be cashed unless equipments are lost or damaged)

• Registration Fee (Check and Money Order Only)

  • $200.00 for Football

  • $100.00 for /Cheerleading

    • ($60.00 Mandatory Camp Fee and approximately $225.00 Uniform Fee)

• Complete Registration Packet

  • Registration Form

    • Code of Conduct

  • Emergency Medical Release Form

  • Parent Volunteer Form

  • Equipment Policy Form (Football Only)

  • Weight Chart

  • Physical Form
